Terms of use
1. Service purpose
OphTrack is a structured ophthalmic surgical training logbook. It is not a patient medical record, medical advice, emergency service, diagnostic tool, or substitute for an employer, university, hospital, regulator, or professional-body record.
2. Access and eligibility
Access is provided only after the request process is completed. Users must provide a valid email address, keep account information accurate, protect their sign-in method, and promptly report suspected unauthorised access.
3. Permitted use
The service may be used for the user’s own lawful training-log purpose. Users must follow applicable professional, institutional, confidentiality, and data-protection obligations.
4. Patient-information prohibition
Users must not enter or upload patient names, initials, contact details, dates of birth, addresses, medical-record numbers, hospital or insurance identifiers, photographs, scans, documents, identifiable narratives, or combinations intended to identify a patient. This prohibition applies even if the user has permission to access that information elsewhere.
5. Prohibited behaviour
- accessing another person’s account or records;
- trying to bypass permissions or security controls;
- probing, scanning, or disrupting the service;
- submitting unlawful, harmful, or malicious content;
- automating abusive requests or attempting to overload the service; or
- using the service in a way that breaches professional secrecy or institutional rules.
6. User content and records
Users remain responsible for the accuracy, lawfulness, confidentiality, and appropriateness of information they enter. Users should keep any record required by their institution or professional body in the format and location required by that organisation.
7. Account restrictions
Access may be limited, suspended, or ended when reasonably necessary for security, misuse, legal compliance, maintenance, or service closure. A user may request deletion or exercise data rights through contact@ophtrack.com.
8. Availability and changes
Features, access rules, and these terms may change during the testing phase. Important changes will be communicated through the service or this page. Availability and error-free operation cannot be guaranteed.
9. Intellectual property
The OphTrack name, visual identity, interface, documentation, and original content are protected. Users receive a limited, personal, non-transferable right to use the service while authorised.
10. Responsibility and liability
Users must not rely on OphTrack as the sole repository for records required elsewhere. Nothing in these preliminary terms limits liability or rights that cannot lawfully be limited.
11. Privacy
Use of the service is also subject to the preliminary privacy information and the GDPR & data-rights information.
