Terms of Use

1. Provider

OphTrack is provided by OphTrack. Contact: contact@ophtrack.com. Hosting information appears in the Legal Notice.

2. Service

OphTrack is a structured ophthalmic surgical training logbook with personal dashboards and CSV export. It is not a patient medical record, emergency service, or tool intended for diagnosis, treatment, or clinical decision-making.

3. Acceptance and eligibility

By accessing OphTrack, you accept these Terms and the Privacy Policy. You must be at least 18 years old and use the service only for a lawful professional or training purpose.

4. Accounts and roles

• Student and Senior accounts may access only their own logbook records.
• Senior status does not grant automatic access to another user's records.
• Admin privileges are reserved for account administration and operational duties.
• You must provide accurate account information and keep credentials confidential.
• Suspected unauthorized access must be reported promptly.

5. Absolute prohibition on patient identifiers

You must not enter or upload:

• patient names, initials, contact details, dates of birth, or addresses;
• medical-record, hospital, insurance, or government identifiers;
• identifiable narratives, photographs, scans, documents, or attachments; or
• any combination deliberately intended to identify a patient.

This prohibition applies even if another person or institution has authorized you to hold the information elsewhere.

6. Acceptable use

You must not misuse the service, probe or bypass security controls, access another account, interfere with availability, submit unlawful content, automate abusive requests, or use OphTrack contrary to professional secrecy or institutional rules.

7. User controls

You may edit and delete individual case records and export your structured logbook. Account-level access, correction, export, or deletion requests can be sent to contact@ophtrack.com.

8. Availability and changes

OphTrack may change, suspend, or discontinue features for security, legal, maintenance, or operational reasons. Reasonable care is taken to maintain availability, but uninterrupted or error-free operation is not guaranteed.

9. Intellectual property

The OphTrack name, software, interface, documentation, and original content belong to OphTrack or its licensors. Users receive a limited, personal, non-transferable right to use the service in accordance with these Terms.

10. Suspension and termination

Access may be restricted or terminated where reasonably necessary to address a Terms breach, security risk, unlawful use, legal requirement, or discontinuation of the service. Users may request account deletion at any time.

11. Responsibility

OphTrack is a supporting logbook and must not be the sole repository for information required by an employer, university, regulator, or professional body. Users should retain required exports. Nothing in these Terms excludes liability that cannot lawfully be excluded.

12. Governing law

These Terms are governed by French law, without limiting any mandatory protections that apply under other applicable law.
